Assignment #1 – due September 30, 2007 Point value: 10
Your CSH team will be completing the School Health Index in September. As part of that process, your assignment is to convene a team of 4–6 school staff to complete at least one module of the School Health Index (SHI). Turn in the module scorecard and planning questions 1–3 from the School Health Index. With your other CSH team members, present your prioritized actions to the building staff.

Assignment #2 – due October 31, 2007 Point value: 15
Your second assignment has two parts:
1. Research best practices in one of four content areas: physical activity/PE; nutrition; tobacco or staff wellness. To find best practices research, download Phase 2 of the Roadmap. Take a look at Step #4 – there are a variety of websites listed that provide best practice research. Feel free to use other research as well.
Complete a referenced fact sheet (download here) indicating three specific strategies that reflect best practices for schools in your selected content area. This is the only document you will submit to Donna for this assignment. Submit it by October 31st via email at: donnad@rmc.org, or fax it to 303-239-8428.
NOTE: Each student must complete this assignment individually. If several of you from the same team are enrolled in this course, please select different content areas. I will post all of your research-based strategies on the website early in November.
2. Using your completed fact sheet, present best practices in your selected content area at the next CSH team meeting.
Assignment #3 – due December 3, 2007 Point value: 25
Work with your Coordinated School Health team to develop a School Health Improvement Plan based on results of the School Health Index. Refer to Phase 2 of the Roadmap for step-by-step guidance.
Turn in a completed School Health Improvement Plan using the template provided (download here). Submit it by December 3rd via email at: donnad@rmc.org, or fax it to 303-239-8428.
The School Health Improvement Plan must also be turned in to your district coordinator, if you have one.
Assignment #4 – due December 31, 2007 Point value: 10
Assignment #4 has two parts:
- Meet with your school team and share your learnings from your best practices referenced fact sheet.
- Submit a progress report by answering the questions on the downloadable December Progress Report. This is the only document you will submit to Donna for this assignment.
Note: If there are several of you on the same school team, you may collectively submit one December Progress Report. Be sure to put your names on the report so all of you receive credit.
Assignment #5 – due February 1, 2008 Point value: 10
Your assignment is to read one of the following books and complete the appropriate worksheet:
Good to Great, Jim Collins – download and complete this worksheet
The Tipping Point, Malcolm Gladwell – download and complete this worksheetSome district coordinators have copies of these books and they are also available in libraries and on Amazon (used copies are inexpensive). This assignment must be completed individually, and students from the same CSH team are requested to read different books.
Happy reading! District coordinators and co-leaders from various school districts have read these books and discovered significant learnings for coordinated school health teams.
Assignment #6 – due February 29, 2008 Point value: 10
Meet with your coordinated school health team to discuss implications for CSH from Good to Great and/or The Tipping Point. Modify your School Health Improvement Plan as needed to incorporate you new learnings.
